Current Practice Fees

We Bulk Bill all valid medicare card holders and DVA card holders.

If no Medicare card, a practice fee applies:

Short Consult: $75
Long Consult: $110

Worker’s Compensation & Motor Vehicle Accident fees:

Level B: $73.65
Level C: $134.60
Level D: $206.75
Private Drivers’ Medicals $77.00
Commercial Drivers’ Medicals $99.00
Missed Appointment Fee $40

Test Results Policy

URGENT RESULTS– Your Doctor or the Practice Nurse will contact you via your telephone number supplied.  If our attempts at contact you are unsuccessful a letter will be forwarded to your address to contact us urgently for an appointment.

NON URGENT RESULTS– Our Nurse will contact you via phone to advise that you need to make a non-urgent appointment to see your Doctor.  If our attempts at contact are unsuccessful a letter will be forwarded to your address to contact us for a non-urgent appointment.

If your results come back NORMAL we do not contact you to advise you of this. You are welcome to phone us to check if your results have been received at the practice and we encourage you to make an appointment to discuss your results with your Doctor.

Please be advised that patient results will not be discussed over the phone due to privacy and confidentiality laws.

Reminder System

Baldivis Family Medical Centre is committed to preventative care, we may send reminders on occasions regarding health services that are appropriate to your care.  If you do not wish to receive health reminders, please advise our staff.


We offer a flexible appointment system with the Doctor of your choice, please advise our friendly Receptionists if you would like a long appointment for a health assessment, medical, worker’s compensation or for any other reason so we can ensure we book your appointment appropriately.

If your medical need is urgent please advise our receptionist as we do offer urgent on the day appointments.

Non attendance Policy

We require a minimum of 1 hours’ notice for appointment cancellation.  If you fail to attend your appointment without adequate explanation, a non attendance fee of $40.00 may be payable.

After Hours Service

When our practice is closed or you require a home visit, please call Perth After Hours Medical Service on 1300 000 362.   This service is fully bulk billed if you hold a valid Medicare card or Veteran’s Affair Card.   If you require urgent medical attention, please call 000.

Baldivis Family Medical Centre protect your personal health information at all times and ensure it is only available to authorised staff members for the intended purposes and we comply with the Privacy Act.

We are committed to protecting your privacy and confidentiality at all times. We have a policy for the Management of your Personal Health Information.

Teaching & Research

We support teaching and research and our practice will actively be teaching Registrars in the near future.
