How to get started:
Entry into the program requires a referral and a Mental Health Treatment Plan from your GP, followed by a brief individual screening with our psychology team. Up to 10 group sessions per year are eligible for Medicare rebates.
Group Availability:
Spaces are limited to a maximum of 10 participants per group. Speak to your GP today or contact our administration desk to express your interest.

Appointments at Baldivis Family Medical Centre can be made online via HotDoc or by calling (08) 9591 8423 You can select your preferred GP and time when booking online. For telehealth consultations, please inform us when scheduling to ensure we allocate the appropriate time.
Billing and Fees
We offer a mix of bulk billed and privately billed services. Fees vary depending on consultation type and Medicare eligibility.

Test Results
It is the patient’s responsibility to follow up on their own test results. Do not assume results are normal simply because you have not heard back.
Results can only be provided by a doctor. Please clarify follow-up arrangements at the time of your appointment.
Referals
To access the specialist Medicare rebate, your GP must first authorise and write a referral. Please see your GP BEFORE visiting a specialist.
Referrals expire after a set period. A further GP review is required to obtain an extension. Back-dated referrals are not issued.
